This form is to be used for submitting requests for reimbursement of Farmington Youth Lacrosse Association (FYLA)-related expenses. Examples include, but are not limited to, the US Lacrosse Coach Registration fee.

steps to submitting your request:

Click on link "Reimbursement Form" link at right and complete the form.
After submitting the form, please email receipt to FYLA treasurer.

By submitting the form, you affirm that the expense was legitimate, directly related to FYLA activities, and approved in accordance with FYLA policies.
